Consumers can use the form below to submit complaints to the North Carolina Office of the Commissioner of Banks.  Since complaints require a signature, the form must be signed and either mailed or faxed to this agency.  The form can be completed on-line and printed out. However, before completing the form, please check that the company is actually regulated by our agency by reviewing our "NC Regulated Financial Institutions" information accessible from our home page.
