Uniform Mortgage Biographical Statement & Consent Form MU2  $46 Control Person(s) and Managing Principal/Qualifying Individual to check credit and criminal background. This form must be completed by each control person as defined in the Form MU1-Explanation of Terms. The Form MU2 can be used to add a control person(s) or to amend an existing control person(s).
Uniform Mortgage Branch Office Form MU3 $125.00 for each new office license ($25 if license is re-issued)

Loan Officers

Obtain or amend license as a Mortgage Loan Officer, permitting the licensee to accept, or offer to accept, mortgage loan applications for North Carolina real estate. No courses will be accepted after April 30 for a current fiscal year (July 1-June 30).

Obtain approval to offer a course to Loan Officer applicants and licensees to meet initial licensing or continuing education requirements. A form must be filed for each course, which may be offered multiple times from July 1 through June 30, whereafter approval for each course must be renewed.
