Mortgage Lenders and Brokers may change their company license type. Listed below are the documents and items needed for the Office of the Commissioner of Banks to process the request.

Please submit the following:

• Original Surety Bond ($150,000 Lenders/$50,000 Brokers)
• License Exchange fee of $625
• Letter detailing the requested license status change
• Balance sheet

If company was Grandfathered we will also need the following items:

• Fingerprint cards for all individuals listed on the Schedule A (page 13/15 on the Form MU1.)
• Authority for Release Form
• Schedule "A"
• Credit & Criminal Fee of $46
